Hola.   Hace 15 días tuvimos la interesante charla sobre la Estimulación del Glanglio de la Raíz Dorsal. Esta exposición nos dio mucha información sobre otras formas de funcionamiento de la estimulación sobre el tejido nervioso. 

Vamos a oír una Charla de 20 minutos sobre que el Doctor Ricardo Vallejo dió en el pasado congreso mundial del World Institute of Pain acerca de los mecanismos de acción de la Estimulación del Cordón Espinal. 

Posteriormente nuestros fellows tendrán 10 minutos para explicarnos acerca de Differential Target Multiplexed™ spinal cord stimulation (SCS). (Medtronic)

LOS FUNDAMENTOS CIENTÍFICOS DEL GANGLIO DE LA RAIZ DORSAL (GRD)

El GRD es una importante estructura de nervios intrarraquídeos epidurales que aloja los núcleos celulares de las neuronas sensitivas primarias (NSP)2.

Estas NSP constituyen la principal fuente de transmisión de información sensitiva desde la periferia hasta el sistema nervioso central, y además representan un importante punto de procesamiento de la información sensitiva1.

El dolor es un tipo de información sensitiva que se transmite y procesa en las NSP dentro del GRD1. En condiciones normales, el dolor representa una importante experiencia sensitiva con fines de protección; sin embargo, en estados patológicos el dolor se convierte en una afección crónica. En las investigaciones se ha demostrado que las NSP alojadas en el GRD desempeñan un papel fundamental en el desarrollo y la persistencia del dolor crónico1. Asimismo, estas neuronas presentan alteraciones en las propiedades de las membranas, como una mayor excitabilidad y una actividad anómala y ectópica3,4.

Si bien estas alteraciones fisiopatológicas contribuyen al estado de dolor crónico, es probable que a su vez permitan que este tipo de células sea más sensible a la influencia de los tratamientos por neuromodulación (estimulación)1.

La clínica del dolor de la Fundación Santa Fe se encuentra actualmente en el proceso de acreditación de un centro en cuidado clínico sobre síndrome doloroso regional complejo. Ese es un proceso largo y exigente en el cual presentamos la evaluación y el manejo de los pacientes con esta patología ante la JCAHO. 

La JCAHO es la organización más importante de Estados Unidos en la acreditación de centros sanitarios. En concreto, se dedica a la inspección, mediante un proceso voluntario de certificación, de unas 20.000 instituciones entre las que figuran hospitales, centros ambulatorios y de atención primaria, centros de salud mental, socio-sanitarios y servicios de atención domiciliaria. Fuera de Estados Unidos, la Joint Commission International tiene acreditados más de 800 centros sanitarios pertenecientes a más de 60 países diferentes.

Metas y objetivos de la JCAHO

La JCAHO tiene una misión explícitamente declarada:

"Mejorar continuamente la atención médica para el público, en colaboración con otras partes interesadas, evaluando las organizaciones de atención médica e inspirándolas a sobresalir en la prestación de atención segura y efectiva de la más alta calidad y valor".

Por esta razón en 10 Minutos unos de los Fellows nos contará que es el Síndrome  Doloroso Regional Complejo, otro en 10 minutos nos hablará de los criterios diagnósticos.  

Finalmente oiremos una muy buena charla  de 20 minutos del Dr Cheng sobre el manejo intervencionista de esta patología.

Hola.   Hoy vamos a hablar de seguridad radiológica en procedimientos intervencionistas en dolor.   Totalmente aplicable también a los anestesiólogos en el quirófano en donde se está usando un fluoroscopio.

Yo hare una breve introducción. 

Luego los fellows hablarán del equipo de fluorsocopio,  las unidades utilizadas para la medición de dosis y finalmente de las herramientas de protección como chalecos, gafas,  guantes, etc.

Esta noche, la doctora Manuela Florez fellow de la Clínica de Dolor nos hablará en una charla de 20 minutos acerca de MINDFULNESS, técnica utilizada desde hace varios años para el manejo, entre otros, del dolor.

Revisaremos el nivel de evidencia que soporta su uso en nuestro campo. 

Posteriormente Carlos Morales, Psicólogo Clínico, experto en el tema nos hablará de experiencia en su aplicación en nuestro pacientes.  

Realizaremos durante 20 minutos un taller.  Les recomiendo para ese momento estar en un lugar tranquilo, dispuestos a participar.  

 Nuestra experiencia con talleres previos ha sido muy satisfactoria.

Iniciaré yo con una charla de 10 minutos en donde les hablaré del sistema endocanabinoide. 

Posteriormente oiremos una magnífica Charla de 30 minutos  de un verdadero experto del tema. Esta Charla se presentó online en el pasado congreso de CAIBDA (Centro de Atención integral del Dolor de Buenos Aires) Agosto 2020.

Como anestesiólogos es  NUESTRA RESPONSABILIDAD autoeducarnos y volvernos expertos en el tema. Como me contaba una colega en Madrid:  “de un momento a otro no solo somos anestesiólogos sino intensivistas, somos la primera linea de manejo en el proceso de intubación en UCI, nuestro quirófano puede ser pronto una habitación de UCI”. 

EL OBJETIVO DE ESTE BLOG ES COMPARTIR INFORMACIÓN QUE HE BUSCADO Y RECIBIDO.  Pretendo ser estricto en mencionar la fuente y si la tengo, la fecha de elaboración de los documentos.

No hay mucho sobre este dispositivo.  En mi opinión es una protección mas que si se usa en conjunto con las medidas de protección aprobadas.  Además les recomiendo que practiquen su uso, lo peor que puede pasar es que improvisen con el y se invierta en un obstáculo.
